Make a list of your current medications from home.
Bring all of your current medications with you.
Please know your diagnosis of your cancer.
Write down the names of the chemotherapy drugs that you are currently receiving.
Keep a current copy of your chemotherapy orders from your medical oncologist.
Keep a current copy of your medical records from your medical oncologist.
Please write down or know your oncologist's name, along with their phone number or email address.
Before leaving, double check and ensure that you have 2 weeks worth of your medicine supply.
